How it Wokrs - Hunt at Home

As a vital, active adult, you're able to enjoy your home, your community and - most important - your independence. But chances are you're concerned about what tomorrow might bring. What if you or your spouse becomes ill or disabled? How would it affect your lifestyle? Your family? Your financial future? Hunt at Home shares your concerns, and we have a solution.

The personalized application process includes a thorough medical assessment. If you have questions or concerns about your eligibility, please contact us to discuss your concerns.

Eligibility
Hunt at Home extends membership to adults aged 62 and older who enjoy good health and who are living independently.

  • Minimum age 62 (no maximum age)
  • Must be living independently and qualify medically (case by case basis, not for individuals needing immediate care) and financially
  • Cannot be oxygen dependent, diagnosed with Parkinson’s, Alzheimer’s or other Neurological Diseases
  • Service area is in Nashua & Designated Surrounding Areas
  • Must maintain health insurance

Care Coordination
You will be assigned a Personal Care Coordinator to coordinate any eligible services you may need.  Your Personal Care Coordinator will work closely with you, your family and your physician to have the right services delivered to you in your home, or when necessary, in a plan-participating facility. 

Home Inspection

When you enroll in the Hunt at Home program, a Care Coordinator will visit you in your home to get to know you – and your surroundings – long before you ever need us.   During the first year of membership and every second year thereafter, Hunt at home will provide a safety and functional inspection of your home, conducted by a licensed occupational therapist, to ascertain any problems and to make recommendations to you based on the inspection.

Annual Physical Examination
Hunt at Home will reimburse you for an annual physical examination (if not covered by Medicare) completed by your personal physician, including labs, x-rays and EKG’s.

Transportation
Hunt at Home will provide a list of providers from a referral base of community providers. You will be responsible for paying for any needed transportation.

Emergency Response System
Hunt at Home will provide an emergency response system with 24-hour coverage when needed.

Eligibility

You are eligible for the following services when you demonstrate a deficiency in one or more of the following Activities of Daily Living (ADL’s)

• Eating
• Dressing
• Grooming
• Transferring
• Toileting
• Bathing
• Walking
• Mobility

Certified Home Health Aide
If you need assistance with your personal care, such as bathing or dressing, a licensed home health aide will be provided for you in your home.  A registered nurse supervises this aide.

Companion Services
A companion/homemaker will be provided when you need assistance in your home in such areas as: supervision of activities of daily living (see above) and/or medication reminders. This caregiver can also do cooking, dishwashing, laundry, light housekeeping and errands, as well as keeping you company with conversation and social activity, while they are providing these services in your home.

Meals
Lunch and dinner can be delivered to or prepared in your home by a companion if you have a deficiency in an ADL.  Delivered meals will be for a short period of time.  1-2 days, if deemed necessary.

Assisted Living
If staying at home ever becomes unsafe, Assisted Living at Hunt Community and The Huntington at Nashua is included for members of Hunt at Home. Assisted Living is utilized when someone needs a lot of assistance with daily living but doesn't need the medical care that a nursing home provides.

Nursing Home

If your health changes and you can no longer remain in your home or in assisted living, nursing home at Hunt Community and The Huntington at Nashua is also included for Members.  Our participating nursing homes provide 24-hour nursing and medical supervision.

Referral Service
You can obtain a referral from Hunt at Home for various types of services you may need to stay in your home. Examples of these services are legal, financial planning, home maintenance, rental of medical equipment, etc. You will be responsible for paying for these services. 

Social and Wellness Programs

These services, provided by  Hunt Community include exercise classes, arts and crafts classes, wellness seminars, speakers and day excursions.  Some programs are free for members of Hunt at Home and others may have a small fee for participation.
